Completing this form:
When you receive this form you will need to insert the following information:

  • Name and address of the parents or legal guardian.

  • Date in which this document is to be executed.

  • Duration (in days) in which the Power of Attorney will be valid.

  • Names and dates of birth of children.

  • Name and address information for the person appointed to be the caregiver responsible for obtaining medical treatment.

Signing and validation:
To make this document valid, the parent or legal guardian should sign the Limited Power of Attorney for Medical Treatment in front of a notary (required by all states).

Additional suggestions:
Once completed you should make a photocopy of your insurance card and prescription card for the caregivers.  This will insure that there are no problems having your child admitted to a hospital, or problems receiving medical treatment from a doctor's office.

Next you should prepare multiple copies of the Limited Power of Attorney for Medical treatment.  That way, the caregiver can leave one copy in their car or office, while taking an additional copy with them wherever they go.
